Hi chapstick!

Thinning skin and loss of fat and collagen become more common as you age, but considering you are only 20, then it might be because your skin is very fair and thin, which is showing the reddish-blue blood vessels under your eyes. We recommend getting evaluated by a qualified plastic surgeon or oculoplastic surgeon for a proper diagnosis. We know this must be very stressful for you.

We can suggest a few options to not necessarily remove, but improve the situation.

There is no such thing as “permanent” treatment for dark circles, they are more long term or semi-permanent.

1) An under eye hyaluronic acid filler is a common and preferred method of helping to treat these types of dark circles. The filler changes how light interacts with the skin and this change causes the dark circle to look much better. These treatments can last well over a year.

2)Undereye fat injection is similar to filler injections, but this requires harvesting fat from your body, and placing and positioning it in the area. This can last longer, but requires a lot more skill and precision, so be sure to choose a clinic who has a consistent and proven track record of performing this procedure effectively. Also, this treatment is more on the pricier side vs traditional fillers.

Other options, which become more involved and surgical are: 1) under eye fat repositioning 2) under eye fat removal or 3) lower blepharoplasty procedures.

Hello, Thank you for your inquiry.

Many people have often expressed a concern regarding dark circles or darkening of the areas below the eyes, which often causes an appearance and impression of being tired or aged. We truly understand your frustration and concern about this.

We took a look at your current photos, and if you are a good candidate for eye surgery, then there are a few surgical options. In regards to the dark circle around your lower eyelid area, it can be improved by transconjunctival blepharoplasty surgery.

Generally, when cosmetic surgery of the lower eyelid is carried out, the fat contained in the fat pocket causing protruding internally is removed or relocated through the conjunctiva without creating a scar for young patients.

In the case of surgery though conjunctiva, there is absolutely no scar that is externally visible and patients can perform their daily life activities a day after the surgery.

Another option that might be suitable is a fat transfer around the lower eyelid. However, if the area is sunken and depressed, then the problem may not be corrected by fat transfer alone.

We recommend having a face to face diagnosis with a qualified plastic surgeon.
